QWEST^® REFER A FRIEND PROGRAM AGREEMENT

I AGREE TO talk to friends, family and businesses about their telecommunications needs.

I AGREE TO obtain the prospective customer's verbal permission to make a referral, as well as some basic information about the customer, including their name, address and billing telephone number; and the services they are interested in.

I AGREE TO use only information that is publicly available to develop referrals and contacts.

I AGREE TO protect customer privacy by adhering to all program Terms and Conditions of the Refer A Friend Program.

I AGREE TO abide by the American Express®-Branded Reward Card Agreement.

I ACKNOWLEDGE that the bank that issues the American Express®-Branded Reward Card complies with Patriot Act requirements.

I ACKNOWLEDGE that I will receive the appropriate tax documentation for cumulative awards over $600 for the calendar year.

I ACKNOWLEDGE that I am not a Qwest employee.

I WILL NOT represent myself as an authorized Qwest sales representative, nor provide customers with price quotes for Qwest products and services. Only a Qwest sales representative can provide exact pricing.

I AGREE TO conduct myself in a professional manner and WILL NOT make any false, misleading or disparaging statements regarding any Qwest competitor or any other individual or organization as it relates to any activity associated with the Refer A Friend Program.

Reward card can be used virtually anywhere American Express Cards are welcome in the USA. Card has a “valid thru” date on the front of the card. After that date, the card cannot be used and is invalid for all purposes. Subject to applicable law, any points remaining on the card after the valid thru date are irrevocably forfeited. Usage restrictions and guidelines apply. Card cannot be redeemed for cash and is not transferable. The card is not for use at taxis, water taxis, limousines and ATMs. The card is point based with 1 point =$1 in purchasing power. Card terms and conditions apply.
